The ideal candidate for the Intake
Coordinator position will demonstrate strong organizational skills, the
ability to communicate well, and be a self-starter.
This position is a part of the quality
assurance team, it involves traveling throughout four counties, to provide
support to all programs. In addition, it involves traveling to other
communities around Iowa to meet individuals interested in IPG services.
The candidate will ultimately display the
compassion and understanding necessary to work with IPG’s served individuals.
Responsibilities of Intake Coordinator
Partnering with the
local programs to complete the intake process from finding referral sources to
coordinating the member’s transition into their new home
Partnering with served
individuals and their teams to develop individualized service plans tailored to
their unique needs and preferences
Reviewing and giving
feedback on medical records and other documentation created throughout the
Support local programs
with updating and filing records as needed
Providing a variety of
support and trainings (especially related to medical records and service plans)
Working knowledge of
Home-Based Habilitation and Waiver Services’ Standards
of Medicaid criteria for documentation
Ability to write
Ability to develop and
write person centered service plans
Ability to give
constructive, respectful feedback, and partner with a variety of individuals to
provide quality services
Educational Requirements: A High School diploma
and a minimum of two years’ experience working with individuals in the served
A four-year college
degree in a human services related field may be substituted for the two years
Salary range: $35,880.00 to $37,960.00 based upon
experience working with adults with disabilities.

LIFE SKILLS SPECIALIST LSS/DSP (Wage scale $11.00 -$12.30 hourly) LSS work directly with our served individuals to provide supports and services. They are a vital part of the team. LSS staff partner with our Members in order to reach and maintain the maximum degree of independence possible for each person. ESSENTIAL JOB FUNCTIONS:
- Ability to communicate and work cooperatively with others.
- Attend all mandatory in-services and other meetings as required.
- Promote a positive agency image at all times
- Provide direct services according to the person’s IPP and Service Delivery Schedule in a manner consistent with agency policy & philosophy.
- Complete documentation of provided services accurately, timely and within agency policy
Valid Drivers License Proof of Vehicle Insurance Diploma/Equivalent Background check**
** All Applicants are expected to pay a onetime $15.00 fee (Payable via cash or Money Order) for the processing of the background check. This fee is refundable after 90 days of employment with IPG.
LSSM-Life Skills Site Manager (Wage scale is $13.25 - $14.50 per hour)
Group, LLC is a local agency which provides in-home services to our neighbors
with an Intellectual Disability, Brain Injury, or Chronic Mental Illness.
The Site Manager
provides direct support to our served individuals and leads the team of Life
Skill Specialists/Direct Support Professionals for that home. Previous
experience working with adults with disabilities is preferred for this
- Ability to communicate and work
cooperatively with others
- Provide direct services according to the person’s IPP schedule in
a manner consistent with agency policy & philosophy·
- Complete documentation of provided services accurately, timely and
within agency policy·
- Responsible for scheduling and ensuring client appointments are
kept according to schedule and assist clients with bill payments and budget·
- Responsible for accuracy of client medication administration
including development of the Medication Administration Record·
- Input into the supervision, coaching and performance evaluation of
Life Skills Specialists·
- Responsible for transportation of served individuals as needed·
- Responsible for ensuring vehicle insurance covers additional
riders and provides proof to supervisor
- Ensures proper condition of home and safety procedures ·
- Complete and submit Incident Reports
- Wage scale is $13.25 - $14.50
